Discharge Planning

accent's clinical team are skilled and experienced in facilitating discharge planning.  Successful discharge planning ensures a smooth transition for the client and family as they return home and to life in the community.

accent has been providing discharge planning assistance for 10 years with an increase in request for these services in the last 12 months.

accent works with inpatient staff closely to sure the person's treatment needs post hospitalisation are identified, the home is safe to return to and that adequate and appropriate support is available to meet client needs.

accent continues to work closely with specialist hospitals including spinal and brain injury units as well as Acute hospitals and Generic rehabilitation centres.  Hospitals that accent has successfully worked with to facilitate discharge planning has included: Westmead Brain Injury Unit, Liverpool Division of Medicine, Liverpool Brain Injury Rehabilitation Unit and RPA Spinal Unit.

Ideally, successful discharge planning occurs when delete:

 Inpatient or Treating Staff:

  • Have identified the complexity of the discharge at an early stage.
  • Have notified the treating doctor and where involved, the managing insurer.

One of these parties then:

  • Engage accent to commence case managing/discharge planning with the person in conjunction with the inpatient/ treating staff.

 accent Rehabilitation Services' Occupational Therapists facilitate:

  • Coordination of home assessment and modifications. This can include sourcing a property for rental or purchase.
  • Long term equipment hire at home.
  • Community services at home.
  • Treatment handover (where required) from inpatient/ treating staff
  • Long term monitoring and follow up.
